SBI account holders can enter their bank passbook by visiting their SBI branch. Some branches of SBI will have pre described forms to issue new bank passbook and some branches will ask you to write a request letter to issue a new passbook.
If your branch is asking you to submit a request letter then you can use the below letter format. You can use these letter formats not only for SBI, you can use them for other banks as well. (Just change the name of the bank and address of the bank)

I Girish Tangudu, working at ABC Pvt Ltd, has a salary account in your bank with a/c no 345XXX234. Hence my account was opened by my employer. I didn’t get any passbook.
So here I am requesting you to kindly provide my bank passbook.
Address the request letter to your bank’s branch manager, write your name, a/c number and request the bank to issue a passbook.
You need to visit your SBI branch where you have an account and request the bank staff to issue a bank passbook for your account.
You should carry a passport size photo of you to the bank and bank staff will affix that photo on the bank passbook and they will authorize it with the bank stamp.
If the bank doesn’t have any crowd, then you will get your new bank passbook immediately after your request. If your SBI branch is crowded, then they will ask you to visit on the next day to collect your passbook.
Bank passbook acts as a proof of address and also proof of your bank details. Whenever somebody asks you to submit your bank details then you can just submit the copy of your bank passbook which consists of all your bank details like your name, a/c number & IFSC code etc.
In metro cities 3000 Rs, in semi urban cities 2000 Rs and in rural areas 1000 Rs minimum balance should be maintained in your SBI account.
Not required. You can update your bank passbook whenever you need a statement.
